About the Role
Edward Hines Jr. VA Hospital seeks a Registered Nurse for the Remote Patient Monitoring (RPM) Home Telehealth Care Coordinator role. This position is part of the Health Informatics unit and requires physical presence at the Hines VAMC.
Key Responsibilities
- Coordinate nursing care focused on patient education and self-management.
- Provide and supervise direct patient care.
- Support veterans through telehealth technologies and disease management.
Requirements
- Registered Nurse license.
