They are responsible for conducting clinical assessments, developing care plans, and collaborating with interdisciplinary teams to ensure seamless care transitions.
Candidates must hold an active state Nurse Practitioner license, national board certification, and a Master’s degree in Nursing. A minimum of five years of nursing experience, including at least two years as a nurse practitioner, is required.
