Key Responsibilities

Manages care of the complex cardiac patient within the scope of the Heart Wellness Program.

Utilizes tele-management to identify problems and avoid hospital admissions and intervenes as appropriate according to Heart Wellness Program protocols.

Manages medication for the inpatient and outpatient population, making necessary adjustment or recommendations in conjunction with the Primary Care Provider or Cardiologist.

Serves as a clinical resource for organization in area of expertise.

Requirements

of 3 years acute care clinical experience in cardiovascular nursing.
