Key Responsibilities
- Manages care of the complex cardiac patient within the scope of the Heart Wellness Program.
- Utilizes tele-management to identify problems and avoid hospital admissions and intervenes as appropriate according to Heart Wellness Program protocols.
- Manages medication for the inpatient and outpatient population, making necessary adjustment or recommendations in conjunction with the Primary Care Provider or Cardiologist.
- Serves as a clinical resource for organization in area of expertise.
Requirements
- of 3 years acute care clinical experience in cardiovascular nursing.
