About the Role

Join a growing MLTC health plan as a Social Work Care Manager in New York. This full-time remote role supports members with medical and psychosocial needs, coordinating care with PCPs and caregivers.

Key Responsibilities

• Develop and monitor care plans.
• Provide case management for medical, social, psychological needs.
• Collaborate with interdisciplinary teams.

Requirements

• 3-5 years clinical social work experience.
• Knowledge of Medicare and Medicaid.
• Experience in interdisciplinary environments preferred.

Benefits & Compensation

• Salary $80,000 to $90,000 annually.
• $15,000 sign-on bonus for NY LMSW or LCSW.
