Key Responsibilities

Care coordinator will assist in identifying patients who would benefit from behavioral health consultation and ensure that patients receive the services that they need.

Requirements

QUALIFICATIONS: Bachelor's degree in public health, community health, psychology, social work or a related field.

Preference for experience and knowledge working with healthcare data to examine process and outcome.

Strong written and verbal communication as well as interpersonal skills to work collaboratively with both patients and clinic staff.

Must have interest in gaining experience and knowledge in trauma informed care.

Salary: $117.6M - $132.5M

Work Schedule

Full-Time position.

Day shift only - no nights.
