  • Care coordinator will assist in identifying patients who would benefit from behavioral health consultation and ensure that patients receive the services that they need.

Requirements

  • QUALIFICATIONS: Bachelor's degree in public health, community health, psychology, social work or a related field.
  • Preference for experience and knowledge working with healthcare data to examine process and outcome.
  • Strong written and verbal communication as well as interpersonal skills to work collaboratively with both patients and clinic staff.
  • Must have interest in gaining experience and knowledge in trauma informed care.
