Key Responsibilities
- Care coordinator will assist in identifying patients who would benefit from behavioral health consultation and ensure that patients receive the services that they need.
Requirements
- QUALIFICATIONS: Bachelor's degree in public health, community health, psychology, social work or a related field.
- Preference for experience and knowledge working with healthcare data to examine process and outcome.
- Strong written and verbal communication as well as interpersonal skills to work collaboratively with both patients and clinic staff.
- Must have interest in gaining experience and knowledge in trauma informed care.

Work Schedule
- Full-Time position.
- Day shift only - no nights.
