Conduct speech, language, cognitive, and swallowing evaluations while developing individualized treatment plans for home health patients. Collaborate with interdisciplinary teams and physicians to provide therapy and complete required OASIS documentation.
Requires an active, unencumbered Florida Speech-Language Pathologist license and a valid driver's license with reliable transportation. Previous home health and OASIS/Medicare documentation experience is preferred.
