Key Responsibilities

• Perform comprehensive physical therapy evaluations, treatment, and related follow-up services, including discharge planning.

• Document patient assessments and pertinent information in a timely manner.

• Provide services and treatments requiring physical therapy skills, according to the written plan of care and the signed plan of treatment.

• Supervise Physical Therapist Assistants and Home Health Aides.

Requirements

• Required 1 Year Physical Medicine/Rehabilitation Experience- Required 1 Year Home Health Experience

• Preferred Physical Therapist (PT) State Licensing Board

• Required Cardiopulmonary Resuscitation (CPR)- American Heart Association

• Required Driver's License
