About the Role

Optimal Care Hospice Chaplains provide spiritual care services to patients and families in hospice, either directly or through coordination with other spiritual counselors.

Key Responsibilities

Conduct spiritual assessments and provide timely services.
Make initial evaluations within 5 days of hospice admission.
Provide follow-up visits as needed.
Participate in individualized care planning.
Provide consultation and education to interdisciplinary teams.
Perform funeral or memorial services as requested.

Requirements

Graduate of accredited seminary or school of theology (BD, MDiv, or equivalent).
Valid driver's license and reliable transportation.
Experience with death, dying, grief, and loss counseling.
Ability to work in interfaith settings and as part of hospice team.
