The Hospice Clinical Social Worker assists the interdisciplinary team in assessing the social, emotional, and spiritual needs of patients and their caregivers. They develop and implement plans of care, provide grief counseling, and facilitate advance care planning to ensure patient dignity and comfort.
Candidates must hold a Master's degree in Social Work or a related field and possess a valid license to practice in the state of operation. At least one year of professional post-graduate social work experience in a healthcare setting is required, with two or more years preferred.
