Home Health Physical Therapist (PRN)

Key Responsibilities

The Physical Therapist will evaluate patients in a home health setting and develop individualized treatment plans to restore function and mobility. They will also provide therapeutic interventions, monitor patient progress, and collaborate with an interdisciplinary team to ensure optimal outcomes.

Requirements

Candidates must hold an active Physical Therapist license in Texas and possess a graduate degree or higher in Physical Therapy. Experience in home health or rehabilitative settings is preferred, along with strong communication skills and knowledge of evidence-based treatment techniques.
