About the Role

Join Atlantic Health System as a Social Worker – Dementia Care Navigator in United States.

Key Responsibilities

will primarily function as one of our GUIDE Care Navigators and acts as the primary point of contact between eligible patients living with dementia, their caregivers (referred to as the dyad), and the care team at the Geriatric Assessment Center and AHS.

Provide eligible patients and families with a screening assessment and educate them about the GUIDE model an.

Provide dementia-specific education and support to the patient and family, and help families learn what to e.

implement strategies to improve medication adherence.

Requirements

Candidates must possess an MSW from an accredited school of social work and hold an active NJ LSW or LCSW license. Previous medical hospital social work and grant experience are strongly preferred, along with demonstrated initiative and strong analytical and communication skills.
