Physical Therapist

Key Responsibilities

The Physical Therapist will conduct comprehensive evaluations and develop individualized treatment plans to improve patient mobility, strength, and functional independence in their homes. They will also coordinate care with an interdisciplinary team and maintain accurate clinical documentation in compliance with Medicare standards.

Requirements

Candidates must hold a degree from an accredited Physical Therapy program and possess a current Michigan Physical Therapy license. Strong clinical assessment skills and reliable transportation are required, with home health experience preferred.
