Key Responsibilities

Independently manage hypertension, CAD, CVA, dyslipidemia (HLD), PVD, PAD

Oversee and adjust anticoagulation therapy, including Coumadin

Perform comprehensive cardiovascular assessments and medication management

Coordinate with internal providers and external referral partners

Requirements

Active, unrestricted license in state of practice

2+ years of cardiology or advanced chronic disease management experience

Strong diagnostic and clinical decision

making skills
