Key Responsibilities

• Areas include: Patient centered clinical case management, to support health and health related outcomes for individuals with complex health & social needs.

• Community based case management and field based visits with members occur in a variety of settings and may include private apartments, motels, correctional facilities, encampments, and shelters.

• Individual reports to Central Office Administration and works closely with an array of partners and providers in the health and social services arena, including state partners.

• Service delivery using a team based care approach, patient engagement tools, care teams, shared care plans, a strength based approach and motivational interviewing skills.
