About the Role

Join Erlanger Health System as a Clinical Pharmacist – Transition of Care in United States.

Key Responsibilities

  • as required Change work conditions based on volume and acuity level of patient population.
  • Daily use of computer/order entry/answering phones.

Requirements

  • Preferred: Board Certification Department Position Summary: Provide Pharmacy review of medication orders prior to administration.
  • Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
  • Participate in the development of medication use management programs within Erlanger Health System including clinical guidelines, critical pathways, disease management and drug use programs.
  • Participate in providing didactic and experiential training in Clinical Pharmacy for pharmacy students and clinical pharmacy residents.
