Key Responsibilities

The Physical Therapist performs evaluations and treatments for patients in their home environment according to physician orders and regulatory standards. Responsibilities include managing patient care plans, documenting progress in EMR, and coaching Licensed Physical Therapy Assistants.

Licensure & Certifications

Requires a current unencumbered license to practice as a Physical Therapist in the state of Ohio and a valid driver's license. Experience in geriatric care is required, while one year of experience in hospital or home health settings is preferred.
