About the Role

Join Devoted as an Advanced Practice Provider (NP/PA) in the Transitions of Care program. Provide virtual post-discharge care to prevent hospital readmissions across Central, Mountain, and Pacific time zones.

Key Responsibilities

Conduct comprehensive post-discharge assessments.
Perform medication reconciliation and place orders.
Manage acute symptoms and chronic disease exacerbations.
Collaborate with interdisciplinary teams.
Participate in quality improvement projects.

Requirements

Active NP or PA license in good standing.
At least 3 years experience in hospital, post-hospital, or primary care.
Active BLS certification.
Ability to work one weekend shift per month.
