About the Role

Join Devoted as an Advanced Practice Provider (NP/PA) in the Transitions of Care program. Provide virtual post-discharge care to prevent hospital readmissions across Central, Mountain, and Pacific time zones.

Key Responsibilities

  • Conduct comprehensive post-discharge assessments.
  • Perform medication reconciliation and place orders.
  • Manage acute symptoms and chronic disease exacerbations.
  • Collaborate with interdisciplinary teams.
  • Participate in quality improvement projects.

Requirements

  • Active NP or PA license in good standing.
  • At least 3 years experience in hospital, post-hospital, or primary care.
  • Active BLS certification.
  • Ability to work one weekend shift per month.
