Key Responsibilities

The social worker is responsible for completing psychosocial assessments and developing care plans to facilitate patient transitions. They also serve as a liaison to the healthcare team to address the emotional, psychological, and social needs of patients and their families.

Requirements

Candidates must hold a Master of Social Work degree and be licensed by the State of Ohio Social Work Board. A minimum of 1-2 years of equivalent experience is required, along with the ability to obtain NCI-APS certification.
