- Conduct prescreening assessments, Evaluating clients’ mental, behavioral, and emotional health Assist in the.
- Provide discharge planning and follow-up for patients assigned.
- Coordinate the interdisciplinary assessment process and develop a bio-psychosocial summary within five (5) days.
