About the Role

Join Ascension as a Social Worker Case Management Pediatrics in United States.

Key Responsibilities

  • Assess patients' needs and coordinate the overall interdisciplinary plan of care from admission to discharge within Case Management departments.
  • Provide support and education regarding resources available to cope with emotional, financial and social needs that arise with a diagnosis.
  • Develop care plans for patients based on assessment findings and consideration of needs and preferences.
  • Assess complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support.
