VA- Pharmacist

Key Responsibilities

The Pharmacist conducts comprehensive medication reviews and collaborates with interdisciplinary teams to optimize therapeutic outcomes. They also support population health initiatives, ensure formulary compliance, and provide clinical recommendations to improve patient safety.

Requirements

Candidates must hold a Doctor of Pharmacy (PharmD) or a Bachelor’s Degree in Pharmacy and possess an active pharmacist license in the assigned state. A minimum of 2 years of clinical pharmacy experience is required, with preference given to those with PGY1 training and experience in medication therapy management.
