About the Role

Join Trinity Health as a Behavioral Health Counselor – PRN, Day Shift in United States.

Key Responsibilities

WILL DO: Demonstrate knowledge and skills to competently care for all assigned age groups Ensure the accuracy of documenting services and supplies provided to the patients Assess patient’s treatment needs and provides appropriate interventions, including but not limited to the following: a) group and family therapy b) risk assessment c) chemical dependency groups d) case management with community providers e) facilitating and documenting a viable discharge plan Complete psychosocial assessment and consult with physicians and hospital staff regarding patient and family psychological issues through formal and/or informal multidisciplinary conferences and treatment planning.
