Senior Social Worker - Onsite

Key Responsibilities

The Senior Social Worker will oversee the Social Services program, supervise staff, and manage resident assessments and care plans. They are responsible for ensuring facility compliance with federal and state regulations while coordinating discharge planning and community referrals.

Requirements

Candidates must hold a Master’s degree in Social Work (MSW) and an active Arizona Social Work license. At least one year of supervisory experience in a healthcare setting is required, along with knowledge of geriatric care and CMS regulations.
