Key Responsibilities

Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes.

Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication

related outcomes for the target population.

Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, considering population

Requirements

Doctor of Pharmacy (PharmD) degree from an accredited institution.

Active pharmacist licensure in the state or region of practice.

Advanced training or certification in population health management or a related field (preferred).

Strong analytical and data interpretation skills.

Work Schedule

Full-time position.
