Key Responsibilities

• Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes.

• Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication

• Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, considering population

Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.

• Active pharmacist licensure in the state or region of practice.

• Advanced training or certification in population health management or a related field (preferred).

• Strong analytical and data interpretation skills.
