Social Worker- Abington Case Management

About the Role

This Social Worker role at Thomas Jefferson University is based at Abington Hospital in Pennsylvania. Key Responsibilities

• Conduct psychosocial assessments and develop intervention plans.
• Provide counseling for patients and families coping with diagnoses and chronic illness.
• Coordinate discharge planning and post-acute care services.
• Facilitate team and family conferences.
• Document all interventions and assessments in the electronic medical record.
• Supervise MSW student placements and mentor new staff.
• Participate in rotating weekend and on-call trauma coverage.

Requirements

• Master's degree in Social Work from an accredited program.
• Minimum 2 years hospital-based social work experience.
• Active LSW or LCSW license preferred for Pennsylvania.

Work Schedule

• Monday to Friday, 8am to 4:30pm.
• Rotating weekend shifts and on-call trauma coverage.
