About the Role

Join Pinnacle Home Care as a Physical Therapist Home Health Fulltime in United States. Now Hiring: Full Time and PRN positions available Service Areas: N. Ft.

Key Responsibilities

  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
  • Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.

Requirements

  • Must have a current PT license (state
