Hospice Social Worker

Key Responsibilities

Provide comprehensive psychosocial services to terminally ill patients and their families, including assessments, counseling, and community referrals. Manage patient cases, participate in interdisciplinary team meetings, and ensure all documentation is completed within required timeframes.

Requirements

Requires a minimum of two years of social work experience in a healthcare or social services setting, with a Bachelor's degree in Social Work (Master's preferred). Must hold a current Social Worker license, a valid driver's license, and valid auto insurance.
