Key Responsibilities

The Adult Behavioral Health Case Manager works with the Behavioral Health department to serve individuals 18+ who have experiences mental health and/or substance use issues.

The Case Manager will work with numerous SIHC department, to include but not limited to medical, dental, and behavioral health to assure wrap around services are being provided to the client.

SPECIFIC DUTIES AND RESPONSIBILITIES: 1.

Provide care and case management for all clients, to include identifying needs, advocating, and connecting individuals to resources by providing warm hand offs and introductions.
