Provide home

based social services to patients dealing with end

Work as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT, or SLP, focused on the patient’s plan of care and individual goals

Monitor a patient’s psychosocial condition and identify social and emotional needs
