Pharmacist - Medication Therapy Management

Key Responsibilities

The pharmacist provides one-on-one medication therapy management consultations via phone or video to identify and resolve medication-related problems. They also collaborate with case managers to develop personal care plans and monitor patient outcomes.

Requirements

Candidates must hold an active Arizona Pharmacist License and a Doctor of Pharmacy (PharmD) degree. Proficiency in Microsoft Office and the ability to work in a deadline-oriented environment are required.
