Join Neighbourly Pharmacy as a Consultant Clinical Pharmacist – Long-Term Care in United States. Who We Are: Janzens Pharmacy is seeking a Full-Time Consultant Clinical Pharmacist (Long-Term Care) in Thunder Bay, ON! Key Responsibilities

  • Perform comprehensive medication regimen reviews for residents, identifying, preventing, and resolving drug-related problems.
  • Collaborate with residents, family members and healthcare team to optimize patient outcomes.
  • Monitor for effectiveness of therapy and adverse drug reactions and recommend dosage adjustments as necessary based on clinical parameters.
  • Ensure compliance with provincial and federal regulations by auditing medication storage, documentation, and destruction in accordance with facility and corporate policies.

Requirements

  • Excellent interpersonal and communication skills for working with a diverse healthcare team.
  • High attention to detail regarding medication orders and documentation.
  • Ability to work independently and manage multiple, complex patient cases.
