Key Responsibilities
- Medication Review: Conduct thorough medication reviews to identify potential drug-related problems, interactions, and contraindications.
- Patient Consultations: Collaborate with healthcare providers to provide medication therapy management, medication reconciliation, and patient counseling.
- Clinical Interventions: Participate actively in interdisciplinary rounds and provide real-time drug information and recommendations to healthcare teams.
- Protocol Development: Assist in developing and implementing clinical pharmacy protocols and guidelines to enhance patient care.
Requirements
- Doctor of Pharmacy (Pharm. D.) degree from an accredited pharmacy program.
- Licensed as a pharmacist in the state of Arizona, in good standing.
- Strong clinical pharmacy knowledge, including pharmacokinetics and pharmacotherapy.
- Excellent communication and collaboration skills.
