About the Role

Optimal Care Hospice Registered Nurses provide comprehensive nursing care, case management, and compassionate support to terminally ill patients in home or facility settings. You will lead interdisciplinary teams and coordinate individualized care plans.

Key Responsibilities

  • Conduct initial patient evaluations and ongoing reassessments.
  • Manage caseload including assessment, planning, coordination, implementation, and evaluation.
  • Perform hospice-specific assessments such as home safety and support system evaluations.
  • Provide compassionate support at time of death and bereavement assessments.
  • Maintain professional communication with physicians, patients, families, and team members.

Requirements

  • Graduate of accredited nursing program (Diploma, Associate, or Baccalaureate).
  • Current Registered Nurse license in Michigan.
  • Minimum 1 year clinical nursing experience.
  • Ability to handle multiple priorities and documentation requirements.
  • Reliable transportation and valid automobile insurance.
