Key Responsibilities

Provide medical social services to patients in their homes, including psychosocial assessments and the development of care plans. Coordinate necessary resources and provide counseling to patients and families to assist with health status changes and discharge planning.

Requirements

Requires a Master's degree in social work and at least one year of experience in medical social work. Must hold a valid, unrestricted professional license, a driver's license, and current CPR certification.
