Social worker

Key Responsibilities

The social worker will assess the psychosocial status of hospice patients and their families to provide counseling, crisis intervention, and support. They will also collaborate with the interdisciplinary team to develop and implement individualized care plans while maintaining accurate clinical records.

Requirements

Candidates must hold a master's degree in social work, though a bachelor's degree is also accepted. A minimum of two years of documented supervised experience in health care or hospice is required.
