Provide direct patient care through office visits and telehealth while developing personalized treatment plans including lifestyle interventions and hormone therapies. Collaborate with a multidisciplinary team to document patient progress in the EHR and participate in quality improvement initiatives.
Requires an active Oregon license as a Nurse Practitioner, PA-C, MD, or DO with board certification in Family or Internal Medicine. A minimum of 2 years of clinical experience is preferred, along with proficiency in electronic health records.
