About the Role

Join Ophelia Health as a Licensed Clinical Care Manager (Nurse Practitioner) providing telehealth care for patients with opioid use disorder (OUD) and select psychiatric conditions. This full-time role supports stable patients with medication and behavioral interventions.

Key Responsibilities

  • Collaborate with prescribing clinicians to develop and adjust treatment plans.
  • Conduct follow-up visits, induction check-ins, and urine drug screen visits.
  • Provide patient education and behavioral health evaluations.
  • Document all patient encounters in the EHR.
  • Support patients with non-judgmental, compassionate care.

Requirements

  • Nurse Practitioner licensed in Pennsylvania (dual NY license preferred).
  • Active DEA license.
  • Comfortable working in telehealth and with vulnerable populations.
  • 1-3+ years post-training experience preferred.
