Social Worker / Hospice - Part-Time

Key Responsibilities

The Social Worker will conduct psychosocial assessments and provide ongoing emotional support to hospice patients and their families. They will also coordinate community resources, assist with advance directives, and participate in interdisciplinary team meetings to ensure holistic care.

Requirements

Candidates must hold a Master's degree in Social Work and possess current state licensure. A minimum of 2 years of experience in a healthcare or social services setting is required, along with a valid driver's license for local travel.
