The Nurse Practitioner manages a defined patient panel with chronic conditions, performing physical exams, ordering diagnostics, and formulating longitudinal care plans. They collaborate with interdisciplinary teams to ensure care alignment and improve patient outcomes while reducing hospital readmissions.
Candidates must hold a Master’s degree in Nursing and possess at least 2 years of experience as a Nurse Practitioner in primary care, cardiology, or nephrology. A current state license, national board certification, and a valid driver's license are required.
