Key Responsibilities

• INCLUDE: Responsible for documentation including, but not limited to: Develop and update treatment plans for all clients.

• Document results of treatment plan reviews for all clients on a semi-annual basis or at the request of parent.

• Responsible for daily/progress notes for therapeutic service hours delivered, as shared with other facilitators within the groups.

• Responsible for weekly summaries of assigned youth within ASP.
