Social Worker (PRN)

Key Responsibilities

The Social Worker will provide psychosocial support to residents and families while coordinating care from admission to discharge. Responsibilities include completing required documentation, facilitating advance directives, and participating in care plan meetings.

Requirements

Candidates must hold a Social Work degree (BSW or MSW) and meet state licensure requirements. A minimum of 3 years of experience working with the older adult population in post-acute or long-term care is preferred.
