- Accept referrals for care coordination services according to the process designated by the Manager, Clinical Social Work in the outpatient medical clinics.
- Utilize protocols to gather information, assess needs, maintain a record of contacts, and provide a record of case activities and status through referral database.
- Complete and submit documentation for the medical record to supervisor for approval in a timely manner.
- Adhere to standards of care and continuing education outlined in the Case Management chapter 10.

- licensed, and/or internship experience. Experience in a medical or mental health setting preferred. Minimum pay range USD $64,000.00/Yr. Maximum pay range USD $72,410.00/Yr.
