Clinical Pharmacist - Ambulatory Endocrinology and Rheumatology

Key Responsibilities

The Clinical Pharmacist is responsible for the medication management of assigned patients, including monitoring therapy, conducting medication reconciliation, and providing patient education. They also contribute to the development of clinical pathways, mentor residents and students, and ensure compliance with regulatory standards.

Requirements

Candidates must hold a Bachelor of Science in Pharmacy and have completed a PGY1 Pharmacy Practice Residency, or possess three years of related specialty experience. A current Pharmacy license in the state of practice is required, along with proficiency in computer applications and strong communication skills.
