About the Role
Join VNA & Hospice of the Southwest Region Inc as a Physical Therapist – Home Health in United States. VNA & Hospice of the Southwest Region is currently seeking a full-time Physical Therapist (PT) for the Kids on The Move program in Franklin County. Are you looking to further your career in home health therapy?
Key Responsibilities
- Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
- Creates patient specific goals and ensures that the plan of care reflects logical interventions and advancement towards these goals.
- Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc.
Requirements
- Successful completion of an accredited Physical Therapy program required.
- Two years of clinical Physical Therapy experience preferred.
- Prior Home Care experience preferred Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency.
- Experienced with electronic medical records required.

- Hourly rate: $60.42/hr.
