The nurse practitioner will assess the medical and mental health needs of long-term care residents to develop and implement proactive care plans. They will also collaborate with an integrated care team, including RN case managers and supervising physicians, to improve patient outcomes and prevent unnecessary hospitalizations.
Candidates must hold an FNP, AGNP, AHACNP, or PA license and possess a degree from an accredited APRN or PA program. A minimum of 3 years of clinical experience in geriatric nursing or rehabilitation is required.
