Key Responsibilities
- Provides the necessary interventions in order to assure effective and efficient clinical resource management.
- Provides intervention regarding adjustment to hospitalization, crisis intervention and psychosocial assessment.
- Will be actively involved in Discharge Planning and Utilization Review in a progressive acute care setting.
Requirements
- related field. PREFERRED Three (3) years of experience in case management, social services or related field. One (1) year of experience in utilization review. Certified Case Manager. Work Hours: 7am
- 7pm Weekend Requirements: Yes On
- Call Requirements: No
Work Schedule
- 3x12 weekends schedule.
- Call responsibilities required.
- Weekend coverage may be required.
