Join Community Physicians as a Geriatric Nurse Practitioner – Sign-On Bonus, Mentorship in United States.

Key Responsibilities

will see patients: In clinic In assisted/independent living In skilled nursing After hospital discharge This provides rare exposure to the full arc of geriatric care.

will manage: Polypharmacy Frailty syndromes Dementia care High-utilizer patients Advanced care planning Chronic disease optimization If you enjoy nuanced, thinking medicine, this is for you.

manage a defined panel of geriatric patients Provide outpatient primary care visits in a geriatric-focused.

Conduct rounds in Assisted Living and Independent Living communities Participate in Skilled Nursing Facility.

Benefits & Compensation

Salary: $120K - $140K

Signing bonus: $20,000.

CME allowance: $2,000.

Work Schedule

Full-Time position.
