Job Description

About the Role

Optimal Care Hospice Registered Nurses provide skilled nursing care and case management to terminally ill patients in home or facility settings. You will lead interdisciplinary teams and coordinate patient care plans in collaboration with the Hospice Medical Director.

Key Responsibilities

  • Complete initial and ongoing patient nursing evaluations.
  • Manage caseload including assessment, planning, coordination, implementation, and evaluation of care plans.
  • Perform hospice-specific assessments such as home safety and support system evaluations.
  • Provide support at time of death and bereavement assessments.
  • Maintain positive communication with physicians, patients, families, and team members.

Requirements

  • Graduate of accredited nursing program (Diploma, Associate, or Baccalaureate).
  • Current Registered Nurse license in Michigan.
  • Minimum 1 year clinical nursing experience.
  • Ability to handle multiple priorities and documentation requirements.
  • Reliable transportation and valid automobile insurance.
