Key Responsibilities

• develop personalized treatment plans that integrate evidence-based practices, tailored to their unique needs.

• Provide individual, family, and/or group therapy sessions using modalities to help clients reduce distressin.

• provide coordinated care and advocate for clients, including referrals to additional resources and support s.

Requirements

• LCSW, LMHC, or LMFT in the State of Indiana required.

• Master’s degree from an accredited college or university in a mental health related field required.

• 2 + years of experience providing mental health treatment required.

• Possess current Indiana Driver’s license with proof of current auto insurance. Patients First
