About the Role

This Social Worker position is assigned to provide social work services to Veterans and their families receiving primary and specialty medicine care at the Montevideo CBOC under the VA.

Key Responsibilities

Conduct behavioral health and mental health biopsychosocial and comprehensive assessments of Veterans.
Provide social work services within the Patient Aligned Care Team (PACT) model focusing on prevention, health promotion, coordination, and chronic disease management.
